Super Members

	Beneficiary Nomination - Superannuation Members Use this form if you want to make or change a beneficiary nomination with Asset Super.
	Change of Personal Details - Superannuation Members Use this form if you wish to update any personal details or change your insurance cover.
	Choice of Super Fund Use this form to nominate your chosen Superannuation Fund.
	Deduction For Personal Super Contributions Use this form to provide your notice of intent to claim or vary a deduction for personal super contributions.
	Insurance cover Use this form to reduce or cancel Insurance cover.
	Investment Strategy Switching - Superannuation Members Use this form to change Investment choice.
	Lost Super Authority Form Use this form if you would like our ‘Super Sleuth’ to search for your lost super.
	Member Direct Debit Request Forms & Service Agreement Use this form to set up a Direct Debit arrangement.
	Personal Contribution Use this form used to submit a Personal Contribution.
	Tax File Number Declaration Use this form to notify of your Tax File Number. Avoid unnecessary taxes and submit your TFN!
	Transfer Form - Superannuation Members Use this form to transfer money from other superannuation funds into Asset Super.

Pension Members
